The Office of Congressman Rudy S. Caoagdan is committed to protecting the privacy and confidentiality of personal information collected through this Online Assistance & Appointment Monitoring Portal in full compliance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012 (DPA).
Key Privacy Commitments:
- Purpose Specification: All personal details, mobile numbers, and Government IDs are collected solely for constituent verification, assistance qualification screening, and SMS schedule notifications.
- No Commercial Use: Your information is never sold, leased, or shared with commercial entities or third parties for promotional purposes.
- Secure Storage: Uploaded Government IDs and contact data are stored in restricted-access directories accessible strictly by authorized Public Assistance Officers.
🪪 Valid Government ID & Screening Policy
To prevent fraud, duplicate claims, and identity theft, every constituent must upload a clear photo or copy of a Valid Philippine Government-Issued ID upon account registration.
Accepted Government-Issued IDs:
- Philippine National ID (PhilID / ePhilID)
- Driver's License (LTO)
- Voter's ID / COMELEC Voter's Certificate
- Philippine Passport (DFA)
- UMID / SSS / GSIS Card
- Postal ID (Digitized)
- Senior Citizen ID / Person with Disability (PWD) ID
- Barangay Certificate with Photo & Indigency Certification (for constituents without national IDs)
Note: The uploaded ID is used for initial eligibility screening. When visiting the Congressional District Office on your scheduled appointment date, please bring the original physical ID and 1 photocopy.
📅 Fair Appointment Scheduling & Slot Policy
To ensure orderly public service and accommodate all constituents across Kidapawan City, Makilala, Magpet, President Roxas, Antipas, and Arakan, the following appointment policies apply:
- 1 Active Appointment Rule: A constituent may have only one (1) active pending or confirmed upcoming appointment at any given time. Once the appointment is completed or resolved, a new appointment may be requested.
- Punctuality: Please arrive 15 minutes prior to your selected time slot at the Congressional District Office in Kidapawan City.
- SMS Confirmation: Submitting an appointment online places your request in Pending status. You will receive an official SMS text message once your schedule is confirmed by staff.
- Cancellations: If you are unable to attend your scheduled date, please cancel your appointment through your constituent dashboard so your slot can be released to other residents in need.
🤝 Congressional Assistance Programs Guidelines
Congressional assistance is extended to indigent, marginalized, and crisis-affected constituents of District 2. The primary programs include:
🏥 Medical Aid
Hospitalization, chemotherapy, dialysis, laboratory tests, and medicines for admitted and out-patients.
🎓 Educational Aid
Tuition subsidies, school supplies grants, and financial assistance for enrolled college and high school students.
🕊️ Burial Support
Mortuary aid and funeral service assistance for indigent families grieving a deceased family member.
🤝 Emergency Crisis Aid
Urgent financial relief for victims of natural disasters, fire incidents, or acute economic emergencies.
🛡️ Account Security & Code Verification Policy
To protect your account from unauthorized access and ensure legitimate communication:
- SMS Code Verification: A 6-digit numeric verification code is dispatched via SMS to your registered Philippine mobile number during registration, password resets, and account unlocking.
- Failed Attempt Lockout: Accounts with five (5) consecutive unsuccessful password attempts are automatically safeguarded. Unlocking requires SMS mobile code verification.
- Credential Confidentiality: Never share your verification codes or password with anyone. Office staff will never ask for your password.
